SDA Participant Housing Statement

Instructions

  • This participant housing statement communicates your housing needs and preferences directly to decision-makers at the NDIA.
  • Try to keep the statement short (1-2 pages).
  • Focus on the reasons why you want/need to move from where you are living now and why you want to live alone.
  • The blue italicised text in each section are examples of what information you could include in your statement. You don’t need to address each dot point, only what you feel is relevant.
  • Type your statements under each bold heading using the prompts provided. When completed, delete these instructions and the examples listed below in blue.
