Our team of experts can clarify housing goals and needs, explore suitable housing options, coordinate and submit funding requests, strengthen existing evidence to improve outcomes, and support tenants through reviews and appeals.

The Home and Living Support Coordination service is only available to NDIS participants aged 18 and above.
The Housing Hub is dedicated to working directly with people with disability to understand their housing needs and preferences. We take a rights-based approach, championing choice, control, and inclusion for people with disability at every step. Our team maintains independence, ensuring no benefit, tangible or otherwise, is received from housing and support providers.
Based on the person's home and living goals, we will tailor an individualised service agreement that aligns with their needs. The costs below outline our funding structure based on the current NDIS price guide.

This is a specialised service designed to strengthen applications before submission to the NDIS.
Through this service, our Home and Living Support Coordination team and Clinical Practice Advisor will review key evidence, identify areas for improvement, and provide expert feedback to enhance the quality and impact of your application.
