Residential Aged Care (RAC) Services Coordination
The Summer Foundation and Housing Hub are collaborating to provide a free service for young people living in residential aged care (RAC).
The RAC Service Coordination service can support NDIS participants living in residential aged care (RAC) to become better informed of the funding and support that may:
- Increase their ability to participate in the community
- Ensure that equipment and therapy are in place to support quality of life
- Support the exploration of moving out of RAC and into the community
The RAC Service Coordinator will work with each individual to support their unique needs.
