From comprehensive assessments to high-quality reports that strengthen NDIS funding outcomes, we focus on achieving meaningful, real-world results that help you live well at home.

This service is available exclusively to NDIS participants aged 18 and over in the western suburbs of Melbourne.

How we can help

  • Deliver therapeutic supports to build skills, confidence, and independence at home
  • NDIS Home and Living assessments and reports (including SDA, SIL and ILO)
  • Complete comprehensive functional capacity assessments to understand your needs, goals, and living situation
  • Assess and recommend Assistive Technology solutions to enhance independence
  • Assess your home for safety, accessibility, and suitability and recommend minor home modifications
  • Strengthen clinical evidence for NDIS reviews and funding decisions

Based on the person's home and living goals, we will tailor an individualised service agreement that aligns with their needs. The costs below outline our funding structure based on the current NDIS price guide.
