General Housing and Support Participant Statement

Instructions

  • This general Housing and Support Statement is designed for people who are asking the NDIS for a change in the way they are currently receiving support. This could be a request for funding for Activities of Daily Living, a SIL application or an ILO application. 
  • When writing this statement, remember that the NDIS does not fund housing (except when Specialist Disability Accommodation is being applied for). Therefore, rather than focusing on the housing, focus on the way the support will work for you within your new or current home. 
  • This participant housing and support statement communicates your needs and preferences directly to decision-makers at the NDIA. 
  • Try to keep the statement short (1-2 pages). 
  • Focus on the reasons why you want/need the support in place
  • The blue italicised text in each section are examples of what information you could include in your statement. You don’t need to address each dot point, only what you feel is relevant. 
  • Type your statements under each bold heading using the prompts provided. When completed, delete these instructions and the examples listed below in blue.
