As a Housing Provider, you know that securing suitable housing can be a complex journey for people with disability. That’s where the Housing Hub's Home and Living Support Coordination team can help. We work alongside NDIS participants to explore housing options, secure the right supports, and strengthen NDIS funding applications, all of which contributes to better tenancy outcomes with more stable housing arrangements.

Referring your current or incoming tenants to use this service ensures that they get specialist and experienced support in their housing journey while also supporting your organisation’s goals of tenancy stability, property suitability, and reduced vacancy rates.

The Home and Living Support Coordination service is only available to NDIS participants aged 18 and above.

Cost

NDIS participants can use their funding to access this service. Each service agreement is tailored to meet the individual’s unique needs and goals. Our work is billed in accordance with the current NDIS Price Guide (subject to change).

  • Specialist Support Coordination: Level 3: $190.54/hr
  • Support Coordination: Level 2: $100.14/ hr
  • Individualised Living Options (ILO): Design and Exploration: $100.14/hr
