Guide: Service Agreement Guide

The following is a guide to working on a service agreement between an NDIS participant and a Housing Provider or support provider. This guide aims to help people to make a service agreement in a way that brings together the voice of the participant and the Provider. 

Please be aware there are differences in how each state and territory manages tenancies. You will need to check this with your Housing Provider.

Share Information

Participants and providers sharing information about each other allows for a better understanding of how to work together. This section outlines the different ways you can share information.

Ask Questions

Participants should think about what to ask and this section provides some suggestions. Providers should encourage questions - see more detail below.

Understand what should be included

This section helps people understand what can be included in a service agreement, when one is required and what the guiding standards are.

Agree

As the service agreement needs to reflect the individual needs of the participant, this section provides templates that may be used and prompts for how the participant's needs can be shown in the agreement. 

Check

This section provides information about rights, responsibilities and how to extend or end an agreement.
