About the service

The Housing Hub Home and Living Specialists are a national team of housing experts home and living supports with a key focus on quality housing outcomes for NDIS participants. Our specialists work collaboratively with the person and their existing team in a targeted and time limited capacity.  

The Housing Hub have partnered with a registered NDIS provider, Verbinding to deliver this specialised service.


What we do

The Housing Hub offers specialised and individualised approaches to NDIS home and living supports.  We acknowledge the specific skillset required to support the home and living journey and are experts at securing positive housing outcomes for our participants.

Our Home and Living Specialists can:

  • Support people to identify their housing goals and housing needs and preferences
  • Explore housing options that align with their housing goal
  • Coordinate and contribute to evidence required for submission to the NDIA in relation to home and living supports. This includes SDA, ILO, and SIL
  • Support with review or appeals required if an NDIS outcome does not align with the person's needs
  • Secondary Consultation - Review and provide feedback on existing home and living evidence to enhance the quality of evidence prior to submission to the NDIA

We put participant preference first. We know that quality housing outcomes are achieved when participants' needs and preferences are front and centre to their housing journey. 

We are the experts. The Housing Hub has been invested in quality housing for people with disability since its inception.  With the introduction of the NDIS, our team has sought to be the subject matter experts and has led several initiatives that have aimed to educate the wider sector in relation to quality housing for people with disability. Our team has been at the forefront of NDIS housing options from the beginning and has expert knowledge of NDIS legislation. We know how to deliver quality reports that evidence eligibility so the person can get a goal aligned funding outcome.


Cost*

Based on the persons home and living goals, we will tailor an individualised service agreement that aligns with their needs. The costs below outline our funding structure based on the current NDIS price guide. 

  • Specialist Support Coordination - Level 3: $190.54/hr
  • Support Coordination - Level 2: $100.14/ hr
  • Individualised Living Options (ILO) - Design and Exploration: $100.14/hr
  • Improved Living Arrangements: $70.87/hr

Home and Living Services Manager

My favourite part about working at Housing Hub is the opportunity to shine light on housing issues across the sector and contribute to change. This is change in the life of one person and change in a system, in a process, in an outcome. I am an Occupational Therapist, specialising in spinal rehabilitation, working more than 10 years within the health sector in both NSW and Victoria. I joined Housing Hub in 2019 whilst completing my Masters in Health Management. I bring to the team knowledge and experience working with people with disability who are exploring long term housing options.

Helping participants of the NDIS achieve their home and living goals is my favourite part of my role. I have a background in case management, having worked predominately within the Justice and Disability sectors, and a broad knowledge of NDIS and mainstream supports and am always working towards helping participants achieve their goals.

Jesse Camilleri

Home and Living Specialist

Assisting people with disability to determine their best housing outcome, and making it happen! I completed a Bachelor of Psychological Science at La Trobe University and began providing behaviour therapy to children with Autism Spectrum Disorder. I then became the Program Coordinator where I managed a team of therapists and liaised with parents and support teams for the children. I then started as a Housing Options Facilitator at Housing Hub, where I ran our SDA Advice Line, in which the range of complex enquiries prepared me for my current role as Specialist Support Coordinator.

My favourite part about working at Housing Hub is getting to help people find home and living situations that they actually want and that are best to support their independence and lifestyles. I have worked in a number of different roles in the disability industry including Support Coordination, NDIS Liaison at the Royal Children's Hospital and Support Work. During my time working as a Support Coordinator I took an interest in the housing space; helping people to move from hospital and group homes into new accommodation.

My favourite part of my role at the Housing Hub is being able to be part of the journey and outcomes that enable people to be able to find their forever home. I have worked in the Disability & NDIS space for over 4 years as a Local Area Coordinator & Support Coordinator. In previous roles, I have supported participants with navigating home and living supports and understand that one of the challenges with this is understanding NDIS process. This is what led me to my role with the Housing Hub as a Home & Living Specialist, where I can support participants and their team to have a clearer understanding of NDIS Housing and support ensuring people with disability can make an informed choice on how they want to live.

Being able to work with team members from all over Australia! As my daughter is also a NDIS participant, working in the disability industry is extra meaningful to me! I find it really interesting to be working with different families with different needs, and also have so much to learn everyday. I’m feeling very fortunate to have the opportunity to work with both hospital and non hospital participants.

I love being part of an organisation that shares my values and passion for helping people find happy and sustainable housing. With a strong background in Community Housing both here and in the UK, I have moved towards a specialism in disability housing. This role is a good opportunity to use and expand on the experience already gained with the Hospital to Home team and continue to support people towards their ideal home.
