Targeted Support Coordination

Our Support Coordination service is focused on quality housing outcomes. If you are at the beginning of your housing journey and want to explore what might suit you, we can support you through this process. We can support you to explore housing options, gather and develop SDA or ILO evidence and submit to the NDIA. Please note that we do not offer long-term or ongoing support coordination.

Through this work, it became clear that many people who may be eligible for SDA find it hard to connect with NDIS support coordinators who have the right experience.

So as well as referring people with disability to experienced support coordinators, we are now also providing specialist support coordination services. Our key area of support coordination focus is quality housing outcomes. This means we can support you to develop your SDA evidence to submit to the NDIS, so that your SDA outcome aligns with your eligibility and preferences. 

We can also deliver support coordination to support you to explore what housing and support options are available and what may be right for you – including Individualised Living Options (ILO) arrangements.

We have partnered with OnSide to deliver our support coordination services. We will look at how these services are working for people with disability over the next six months.


What we’re offering

The Housing Hub team have extensive experience in understanding housing options under the NDIS and supporting people to explore housing options, obtain funding (including SDA and ILO) and identify housing solutions.

We are housing experts and we are now offering that expertise to people with disability through the provision of support coordination.

Our team has partnered with OnSide to deliver you a support coordination service that focuses on your housing journey. The collaboration is delivered under OnSide’s registration with the NDIS Quality and Safeguards Commission.

Secondary consultation

We can also offer support to therapists who may need assistance to develop SDA assessments and/or support in the submission of the overall ‘package’ of information that is intended as evidence of SDA for the NDIS.

We are a team of people focused on quality housing outcomes for participants.  Our expertise is in SDA eligibility and housing options for NDIS participants and we work in collaboration with participants and their supporters to get the best result.

Melody Carbarns

Housing Hub Services Manager

Melody Carbarns is the Housing Hub Services Manager at the Summer Foundation. Her focus is on supporting project coordinators to connect with people with disability who are seeking specialist disability accommodation (SDA).

Melody works on the establishment and implementation of projects that see people with disability connecting with SDA providers, ultimately finding sustainable housing solutions. Melody started with the Summer Foundation in 2016 and has more than 19 years’ experience in the disability support sector. Throughout her career she has worked as a support worker, residential aged care worker, service manager, care coordinator and team leader.

Melody has a Diploma of Community Welfare Work, Bachelor of Business and Arts (in progress) and a Certificate IV in Disability Community Services.

Jesse Camilleri

Housing Options Facilitator

Jesse is a Housing Options Facilitator in the Housing Hub team. He is an SDA expert and your first point of contact on the SDA Advice Line – 1300 61 64 63. He can help out with information about SDA, put you in touch with existing support coordination services in your area and give you more information about our support coordination services.

Jesse has a background in Psychology and comes to the Housing Hub from the ABA program supporting children with Autism. He holds a Bachelor of Physchological Science (Honours) from La Trobe University.

Tony Herbert

Specialist Support Coordinator

Tony Herbert is working with the Housing Hub on the rollout of the new specialist support coordination program.

A small business owner who has worked in a range of disability roles, Tony brings a wealth of sector experience to his role. His most recent work before joining the Housing Hub was as an independent support coordinator with a focus on specialist disability accommodation (SDA) applications.

Rachel Low

Specialist Support Coordinator (SSC)

Rachel assists participants to explore housing options and put together applications that are suited to individual needs and preferences, with a particular focus on Specialist Disability Accommodation (SDA).

Rachel’s role is part of a pilot program at the Housing Hub, which will enable eligible NDIS participants to utilise their Capacity Building funding to access support coordination services under the Specialist (Level 3) stream.

Rachel has a background in community services and has worked in the not-for-profit and local government space. Her experience ranges from family support work, community-based Aged Care, through to supporting people affected by the criminal justice system and social housing tenants with complex needs.
