Since it was founded in 2006, the Summer Foundation has been invested in great housing outcomes for people with disability. Quality housing for people who have high and complex support needs is one of the key ways to solve the issue of young people living in aged care.
Over the years, the Summer Foundation has been working in a variety of ways to improve housing outcomes for people with disability and since the establishment of the NDIS, has been a key player in educating the sector in relation to Specialist Disability Accommodation (SDA). This has included establishing the Housing Hub website and an associated tenancy matching service in 2017.
Through this work, it became clear that many people who may be eligible for SDA find it hard to connect with NDIS support coordinators who have the right experience.
So as well as referring people with disability to experienced support coordinators, we are now also providing specialist support coordination services. Our key area of support coordination focus is quality housing outcomes. This means we can support you to develop your SDA evidence to submit to the NDIS, so that your SDA outcome aligns with your eligibility and preferences.
We can also deliver support coordination to support you to explore what housing and support options are available and what may be right for you – including Individualised Living Options (ILO) arrangements.
We have partnered with OnSide to deliver our support coordination services. We will look at how these services are working for people with disability over the next six months.
We can support you to submit SDA and ILO requests to the NDIS. We can also support you to explore your housing options. If you think you may be eligible for SDA, you are looking for an ILO arrangement or you are at the beginning of your housing journey and want to explore what might suit you, we can support you.
We can also help if you think the SDA outcome you have is not quite right. If you are going to appeal your SDA decision, we can review your evidence and guide you through the process.
Please note that we do not offer long term or ongoing support coordination.
We can also offer support to therapists who may need assistance to develop SDA assessments and/or support in the submission of the overall ‘package’ of information that is intended as evidence of SDA for the NDIS.

Some of these include our SDA Supply Reports and other research, our tenancy matching service, Upskill, our housing brokerage service, collaborative discharge approach and our housing outcomes work.
Now we are offering fee-for-service support coordination to participants, whilst ensuring our other initiatives continue to build the capacity of the sector.
Our team has been at the forefront of SDA and ILO from the beginning and has expert knowledge of the legislation underpinning the funding.
We understand SDA eligibility and can assist you to quickly work out whether to apply for SDA funding and/or look at other options. We also know how to deliver quality reports that evidence SDA eligibility so you get the right funding outcome.

Melody Carbarns is the Housing Hub Services Manager at the Summer Foundation. Her focus is on supporting project coordinators to connect with people with disability who are seeking specialist disability accommodation (SDA).
Melody works on the establishment and implementation of projects that see people with disability connecting with SDA providers, ultimately finding sustainable housing solutions. Melody started with the Summer Foundation in 2016 and has more than 19 years’ experience in the disability support sector. Throughout her career she has worked as a support worker, residential aged care worker, service manager, care coordinator and team leader.
Melody has a Diploma of Community Welfare Work, Bachelor of Business and Arts (in progress) and a Certificate IV in Disability Community Services.
Jesse is a Housing Options Facilitator in the Housing Hub team. He is an SDA expert and your first point of contact on the SDA Advice Line – 1300 61 64 63. He can help out with information about SDA, put you in touch with existing support coordination services in your area and give you more information about our support coordination services.
Jesse has a background in Psychology and comes to the Housing Hub from the ABA program supporting children with Autism. He holds a Bachelor of Physchological Science (Honours) from La Trobe University.
